Insurance Information

At Stevenson & Fairchild OMS we make every effort to provide you with the finest care and convenient financial options. To accomplish this, we work hand-in-hand with you to verify your available insurance for covered procedures.

We will be happy to help in any way we can in filing your insurance claims and handling insurance questions from our office on your behalf.

Our insurance experts will work with you to attain the maximum benefits to which you are entitled.

To expedite this process, please provide our office with a completed insurance form or a copy of your insurance card with the address to which the claim should be submitted.

Surgical fees under $450 require payment in full at the time of service; however, we will gladly file the provided insurance for your reimbursement.

We offer appointments Monday through Friday.

Financial Information

We provide our patients with exceptional care and make every effort to maintain reasonable fees. For your convenience we accept Visa, Master Card, Discover and American Express credit cards. We also participate in the Care Credit payment program. Payment is due at the time of service unless other arrangements have been made in advance. If you have insurance (and your surgical fees exceed $450) we will be happy to check your available coverage, at which time we may accept a partial payment on the day of surgery. Please contact our office if you have any questions regarding payment options or require detailed financial information. Our highly trained staff will gladly answer any questions you may have. Most insurance companies will respond to your claim within four to six weeks. Please call our office if your billing statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can help you make payment arrangements for any outstanding balance through Care Credit if necessary.

Please remember that ultimately you are responsible for all fees incurred by our office regardless of your insurance coverage.
